Job summary

AdventHealth in Orlando, Florida seeks a skilled Care Manager to provide discharge planning and holistic patient advocacy. You will assess discharge needs across inpatient, observation, and ED settings, coordinate with post-acute services, and ensure timely care progression while aligning with social, financial, and family considerations.

You will collaborate in rounds with a multidisciplinary team, document evaluations and barriers, and educate patients and families to empower informed health

• Develops discharge plans with contingency plans throughout the hospital stay.
• Communicates with post-acute services to ensure continuity of care and accurate records.
• Participates in multidisciplinary rounds to align resources and care levels.
• Educates patients and families about illness impacts and resource access; advocates for patient rights.
