AdventHealth in Orlando is seeking an experienced Nurse Discharge Planner to actively participate in multidisciplinary rounds, assess patient and family needs, and develop comprehensive discharge plans. You will coordinate with care teams to ensure timely transitions and educate families on post-acute resources.

The role requires an RN license, 2+ years in hospital nursing, and strong skills in leadership, analysis, and communication.

Qualifications

• RN licensed in Florida or eligible for licensure.
• Experience in discharge planning and care coordination preferred.
• Knowledge of CMS Conditions of Participation for Discharge Planning.

Responsabilités

• Lead multidisciplinary rounds to review patient status and discharge plans.
• Educate patients and families about post-acute care resources and costs.
• Assess readmission factors and optimize discharge timelines.
• Organize and facilitate family care conferences with the team.
• Document discharge plans, barriers, and progression in medical records.
• Coordinate with payors for post-acute authorization as needed.
• Ensure timely transition of care and continuity across inpatient to post-acute settings.
