Responsibilities

• The Discharge Planner ensures effective communication between hospital services and the community for patient discharge.
• Oversees discharge flow from inpatient to safe transition to community/next level of care.
• Assists Social Services with discharge planning including aftercare arrangements.
• Arranges referrals, follow-up appointments, transportation, and related logistics.
• Tracks discharge data and compiles discharge reports and data.
