Qualifications

• Minimum of one year experience in related disciplines with hospital experience preferred.
• Handle With Care training within 30 days of employment; may require flexible hours and overtime.

Responsibilities

• Communicate with referral source within the first 24 hours of admission.
• Identify all outside professionals involved to complete discharge plans and minimize communication duplication.
• Identify needs of referral sources and professionals during hospitalization.
• Oversee ongoing communication of treatment progress, discharge plans, and transfer of documents to outside agents.
