Job summary

University Behavioral Health of Denton is seeking a Discharge Planner to coordinate discharge planning and ensure continuity of care after hospitalization. This freestanding psychiatric hospital serves North Texas with inpatient and outpatient programs, prioritizing patient-centered care.

The position involves collaborating with care teams to plan post-discharge care, communicating with outside professionals, and ensuring timely transfer of required documents.

Qualifications

• Bachelor's Degree from an accredited college or university in Social Work or a clinical related mental health field preferred.
• Minimum of one year experience in related disciplines with hospital experience strongly preferred.
• Handle With Care training within 30 days of employment and prior to assisting in a restraining procedure.

Responsibilities

• Communicate with referral source within the first 24 hours of admission.
• Identify all outside professionals currently working with the patient and facilitate discharge plans while minimizing duplication.
• Identify needs of referral sources and/or professionals during hospitalization.
• Oversee ongoing communication of treatment progress, discharge plans, and transfer of documents to outside agents.
