Job summary

University Behavioral Health of Denton is a private freestanding psychiatric hospital providing mental health and chemical dependency care in North Texas. The Discharge Planner coordinates discharge planning, ensuring continuity of care post-hospitalization and developing care plans without supervisory duties.

Qualifications include a Bachelor's degree in Social Work or related field and at least one year of hospital experience.

Qualifications

• Bachelor's Degree from an accredited college or university, in Social Work or a clinical related mental health field preferred.
• Minimum of one year experience in related disciplines with hospital experience strongly preferred.
• Handle With Care training within 30 days of employment and prior to assisting in a restraining procedure.

Responsibilities

• Communicate with referral source within the first 24 hours of admission.
• Identify all outside professionals currently working with the patient to complete discharge plans and coordinate communication.
• Identify needs of referral sources and/or professionals during hospitalization.
• Oversee ongoing communication of treatment progress, discharge plans, and transfer of documents to outside agents.
