Job summary

Qualifications

• Bachelor's degree required or preferred in social work or clinical mental health field.
• Minimum of one year experience in related disciplines with hospital experience strongly preferred.
• Handle With Care training within 30 days of employment.

Responsibilities

• Communicate with referral source within the first 24 hours of admission.
• With Care Center, social workers, and nursing identify all outside professionals involved to complete discharge plans and coordinate communications.
• Identify needs of referral sources and professionals during hospitalization period.
• Oversee ongoing communication of treatment progress, discharge plans, and timing of discharge to outside parties and transfer required documents.
