Job summary

La Palma Intercommunity Hospital is seeking a Case Management professional to coordinate activities and assist with discharge planning, under the guidance of a Case Manager/Social Worker. The role requires knowledge of discharge planning and utilization management terminology and experience with hospital case management processes.

Qualified candidates will have a high school diploma; an Associate degree or LVN license is preferred, and a Basic Life Support certification is preferred upon hire.

Qualifications

• Knowledge of Discharge Planning/ Utilization Management / Case Management terminology and functions.
• Experience in basic to intermediate computer skills/knowledge.
• High School Diploma required.
• Current BCLS certificate required upon hire and maintain current preferred.
• Associates degree or LVN license preferred.
• Experience in the hospital setting preferred.

Responsibilities

• Coordinate the activities of the Case Management Department under the direction of the assigned Case Manager/Social Worker; assist with discharge plans.
