Job summary

La Palma Intercommunity Hospital in California is seeking a qualified Case Management Coordinator to join our Case Management Department. You will coordinate discharge planning, assist with referrals and authorizations, and help develop patient-focused discharge plans under the Case Manager/Social Worker guidance.

The role requires knowledge of discharge planning, utilization management and basic computer skills, with a high school diploma required and an associates degree or LVN license

Qualifications

• Knowledge of Discharge Planning/ Utilization Management / Case Management terminology and functions in managed care and non-managed care environments.
• Experience in basic to intermediate computer skills/knowledge.
• High School Diploma required.
• Associates degree or LVN license preferred.
• Experience in the hospital setting preferred.

Responsibilities

• Coordinate activities of the Case Management Department under the Case Manager/Social Worker guidance.
• Assist with development and implementation of discharge plans.
• Assist with case management referrals and authorizations.
• Maintain Medicaid state submission forms and file per facility procedures.
• Assist with discharge planning by interviewing patients/families and gathering data.
