Job summary

You will assist with referrals, authorizations, and data collection to develop safe patient-focused discharge plans.

The role requires knowledge of discharge planning terminology, basic to intermediate computer skills, and a high school diploma.

Qualifications

• Knowledge of discharge planning/utilization management/case management terminology and functions.
• Experience with basic to intermediate computer skills.
• High School Diploma required.

Responsibilities

• Coordinate activities of the Case Management Department under the direction of the Case Manager/Social Worker.
• Assist with development and implementation of discharge plans.
• Maintain Medicaid state submission forms and coordinate referrals and authorizations.
