La Palma Intercommunity Hospital is seeking a dedicated Case Management Coordinator to oversee discharge planning activities and assist with referrals, authorizations, and Medicaid form submissions. The role supports a patient-centered approach, ensuring safe discharge plans and collaboration with the Case Manager and social work team.

Qualified candidates will have knowledge of discharge planning terminology, basic computer skills, and a high school diploma.

Competenze

• Knowledge of Discharge Planning/ Utilization Management / Case Management terminology and functions.
• Experience with basic to intermediate computer skills.

Mansioni

• Coordinate activities of the Case Management Department under the direction of the Case Manager/Social Worker.
• Assist with development and implementations of discharge plans and with referrals and authorizations.
