Central Florida Behavioral Hospital, a 174-bed private psychiatric center in Orlando, FL, seeks a Discharge Planner to ensure safe transitions from inpatient care to the community. The role bridges clinical care and aftercare, coordinating services to maintain continuity and reduce readmission risk.

Responsibilities include arranging follow-up services, meeting with patients to review discharge plans, ensuring medication access, and involving families in planning with clear explanations.

Qualifications

• Level 2 Background Investigation is required (information at the provided link).
• Bachelor’s Degree in social work or related field required.
• Two years’ experience in a psychiatric setting preferred.
• Handle with Care, Verbal Deescalation and BLS/CPR required (may be obtained upon hire).
• Effective oral and written communication skills.
• Must be available to attend full hospital and departmental orientations.

Responsibilities

• Arrange and follow up on post-discharge services such as therapy, medication management, housing, and community support.
• Meet with patients to review the discharge plan and ensure patient understanding.
• Ensuring patients leave with prescribed medications and refills, and understanding how to access them.
• Involving patients and their families in planning, providing education, and addressing questions or concerns in understandable language.
