Job summary

Medical Behavioral Hospital of Clear Lake, part of NeuroPsychiatric Hospitals, is seeking a Discharge Planner to join our Social Services team to support patients at our Clear Lake, TX facility. The Discharge Planner coordinates with the interdisciplinary team and patient families to develop complex discharge plans.

You will work closely with physicians, social services, nurses, and other staff to ensure effective transitions, compliant documentation, and optimal use of resources.

Qualifications

• High School Diploma or GED required; Bachelor's degree in Behavioral Health, Social Work, or Human Services preferred.
• Excellent verbal communication skills.
• Ability to work as part of a multidisciplinary team; maintain confidentiality and compliant documentation.

Responsibilities

• Prepares Social Service Discharge documentation for next level of care providers.
• Sets discharge appointments and arranges transportation as necessary.
• Knowledge of behavioral health resources and community services for discharge planning.
• Collaborates with families and outside agencies to ensure seamless transitions to the community.
• Maintains up-to-date discharge planning information based on team input.
• Ensures compliance with hospital policies and federal/state regulations.
