Qualifications

• Associates degree or equivalent experience required.
• BSW preferred.
• 3–5 years of experience preferred.
• Bilingual Spanish highly desirable.
• Must have a valid Arizona driver’s license.
• Experience with culturally diverse, chronically homeless individuals preferred.

Responsibilities

• Lead discharge planning as part of the clinical team.
• Attend daily team huddles, clinic staff meetings and team meetings.
• Maintain documentation in medical records for discharge plans.
• Develop comprehensive discharge plans toward permanent housing options.
• Connect patients to community agencies for services (vocational rehab, mental health, substance abuse, job readiness, parenting).
• Accompany patients on site visits to clarify discharge plans.
• Counsel patients on entitlements and assist with applications.
• Track progress toward discharge and report to supervisor.
• Collect data for grants and projects as directed by supervisor.
