EMPACT-SPC, part of La Frontera family, is seeking a Discharge Planner in Maricopa, AZ. You will coordinate case management for Seriously Mentally Ill adults and help formulate discharge plans with treatment teams in inpatient settings.
Minimum 2 years of relevant experience, a Bachelor's in a behavioral field, and a valid AZ driver's license are required. Bilingual Spanish is a plus; strong multi-tasking and crisis-appropriate demeanor are essential.
Competenze
- Bachelor's degree in behavioral field or equivalent per AZDHS/BHS.
- Minimum 2 years of case management or inpatient behavioral health experience.
- FingerPrint clearance card or able to obtain one.
- Valid AZ driver license with clean record in past 3 years.
- Regular access to reliable transportation.
- Bi-lingual Spanish is a plus.
- Must be 21 years of age or older.
Mansioni
- Provide case management and discharge planning for Seriously Mentally Ill adults.
- Participate in inpatient staffing and treatment meetings for discharge planning.
- Assist outpatient team with Court Ordered paperwork for admissions/discharges.
- Ensure paperwork is completed accurately and timely with Court Coordinator.
