Job summary

EMPACT-SPC, part of La Frontera family, is seeking a Discharge Planner in Maricopa, AZ. You will coordinate case management for Seriously Mentally Ill adults and help formulate discharge plans with treatment teams in inpatient settings.

Minimum 2 years of relevant experience, a Bachelor's in a behavioral field, and a valid AZ driver's license are required. Bilingual Spanish is a plus; strong multi-tasking and crisis-appropriate demeanor are essential.

Qualifications

Bachelor's degree in behavioral field or equivalent per AZDHS/BHS.
Minimum 2 years of case management or inpatient behavioral health experience.
FingerPrint clearance card or able to obtain one.
Valid AZ driver license with clean record in past 3 years.
Regular access to reliable transportation.
Bi-lingual Spanish is a plus.
Must be 21 years of age or older.

Responsibilities

Provide case management and discharge planning for Seriously Mentally Ill adults.
Participate in inpatient staffing and treatment meetings for discharge planning.
Assist outpatient team with Court Ordered paperwork for admissions/discharges.
Ensure paperwork is completed accurately and timely with Court Coordinator.
