Responsibilities

• Connect patients to community agencies for services (vocational rehab, mental health, substance abuse, job readiness, parenting).
• Accompany patients on site visits to clarify discharge plans.
• Counsel patients on entitlements and assist with applications.
• Track progress toward discharge and report to supervisor.
• Collect data for grants and projects as directed by supervisor.

Summary of Position: The purpose of this position is to provide leadership to the clinical team in planning a patient’s discharge. The case manager is the member of the clinical team who is responsible for identifying the needs of the patient as they prepare for discharge, and is responsible for identifying resources in the community that may be used to meet those needs. Case managers assist the patient in breaking the cycle of homelessness by connecting them to any available housing options for which they may qualify.
