Job summary

Valley Health's Winchester Medical Center is seeking a Discharge Planner to assess psychosocial needs and coordinate safe, timely discharges for medically and socially complex patients. You will develop individualized care plans and navigate community resources with a multidisciplinary team to support transitions of care.

The role requires a Bachelor’s (mandatory) and preferably a Master’s in Social Work, along with strong communication skills and experience in a medical setting.

Qualifications

• Bachelor’s degree from an accredited social work program or equivalent required.
• Master’s degree from an accredited social work program or equivalent preferred.
• Previous social work experience in a medical setting preferred.

Responsibilities

• Coordinate patient care with clinical partners to achieve outcomes.
• Develop and communicate discharge plans for medically complex cases.
• Conduct psychosocial assessments and provide crisis intervention.
• Educate on community resources and assist with referrals.
• Coordinate guardianships, advance directives, and protective services.
• Ensure documentation and communication with care teams and families.
