Valley Health in Winchester, VA, seeks a Discharge Planner II to assess patients holistically and create individualized care plans addressing social, emotional, medical, spiritual, and physical needs. You will coordinate with interdisciplinary teams to ensure safe, timely discharges and provide psychosocial support and resource navigation.

Competenze

• Bachelor’s degree from an accredited social work program.
• Master’s degree from an accredited social work program preferred.
• 2 years of previous social work in a medical setting is required.
• BLS Certification (AHA Healthcare Provider) required.
• Ability to recognize, identify, and report signs of abuse/neglect.
• Knowledge of Medicaid processes for Virginia and West Virginia.

• Coordinate patient care with clinical partners to achieve realistic outcomes.
• Develop, implement, and communicate discharge plans for medically complex cases.
• Facilitate interdisciplinary care planning meetings and delegate tasks.
• Coordinate legal processes for guardianships, advance directives, and protective services.
• Educate patients and families on community resources and referrals for continuity of care.
• Ensure compliance with Medicare, Medicaid, and insurance requirements; complete Medicaid screening paperwork.
