Job summary

Valley Health is seeking a Discharge Planner-Licensed Practical Nurse in Winchester, VA, to support RN Case Managers in organizing thorough discharge plans. The LPN will complete clinical assessments for Medicaid services in multiple states and handle pre-screens, admissions, and insurance authorizations for skilled nursing and transitional care beds.

Requirements include Virginia LPN licensure, two years bedside nursing experience, and BLS certification.

Qualifications

• Graduate of an accredited nursing program is required.
• Two years bedside nursing experience is required.
• Licensed Practical Nurse in VA is required with BLS/HCP certification.

Responsibilities

• Assist RN Case Managers with discharge planning and coordinating post-acute services.
• Complete pre-screens, admissions, and insurance authorizations for Medicaid-related beds.
• Interviews with patients and families; maintains communication with care management for approvals/denials.
• Assist with Level 2 (MIMR) assessments and arrange community resources for seamless transitions.
