Job summary

Fairview Skilled Rehabilitation & Nursing Center is seeking a compassionate Discharge Planner / Social Worker to coordinate safe and timely discharges for our residents. You will collaborate with residents, families, physicians, nursing, therapy, and community providers to ensure smooth transitions to the next level of care.

Qualifications

• MSW preferred; LMSW/LCSW preferred.
• Experience in skilled nursing, rehabilitation, hospital, or case management preferred.
• Strong communication, organizational, and teamwork skills.
• Knowledge of Medicare, Medicaid, and discharge planning is a plus.

Responsibilities

• Coordinate discharge planning from admission through discharge.
• Complete psychosocial assessments and provide emotional support to residents and families.
• Arrange home care, medical equipment, transportation, and community services.
• Communicate with hospitals, physicians, insurance companies, and outside providers.
• Maintain accurate documentation and ensure compliance with state and federal regulations.
• Participate in care plan meetings and collaborate with the interdisciplinary team.
