Job summary

Qualifications

• Bachelor’s degree in Social Work or similar field required.
• Strong written and verbal communication skills.
• Effective interpersonal skills and ability to work independently.

Responsibilities

• Assist with initial discharge planning assessment and identify post-discharge needs.
• Coordinate post-acute patient placement including SNF referrals and home health services.
• Document important message issuance for Medicare patients and maintain patient records.
• Assist with obtaining medications for post-acute care and arrange procurement.
• Participate in multidisciplinary rounds to review plan of care and utilization management.
• Provide patient/family with information about care placement options and obtain consents.
• Maintain knowledge of community resources and advocate for patient/family throughout care episode.
• Assist with documentation and tracking department metrics.
