Job summary

Terros Health seeks a Discharge Planner to bridge inpatient care with outpatient services, focusing on Level I GMH/SA transitions. You will connect inpatient providers with outpatient clinicians, reviewing clinical data to arrange appropriate community resources and follow-up.

Collaborate with hospital teams, PCPs, and RBHA care management to ensure safe discharge, reduce readmissions, and support continuity of care across Terros Health programs.

Qualifications

• Experience coordinating discharge planning across inpatient and outpatient settings.
• Ability to review clinical information and identify resources for safe discharge.
• Maintain confidentiality of client records at all times.

Responsibilities

• Discharge Planner activities performed in inpatient and outpatient settings.
• Coordinate transition from inpatient to TERROS Outpatient treatment post-discharge.
• Collaborate with hospital staff, PCPs, health plans and RBHA care management for smooth transitions.
• Ensure effective coordination to reduce readmissions and crisis service utilization.
• Maintain professional communication and confidentiality.
