Job summary

Terros Health is seeking a Discharge Planner to bridge inpatient care with outpatient services. You will review clinical information, identify resources for post-discharge care, and coordinate transitions with hospital staff, care management, PCPs and health plans.

The role emphasizes reducing readmissions and crisis service use, ensuring a seamless handoff to Terros outpatient care while maintaining client confidentiality and professional collaboration.

Qualifications

• Minimum four years of behavioral health education and work experience.
• Bachelor’s degree in any field with one year of behavioral health experience.
• Bachelor’s degree in a behavioral health field.
• Must have strong interpersonal and communication skills.

Responsibilities

• Discharge Planner activities performed in inpatient and outpatient settings.
• Connect individuals with ongoing outpatient treatment after discharge.
• Coordinate with hospital staff, RBHA care management, PCPs and health plans.
• Aim to reduce readmission and crisis service usage, and ensure smooth transition.

The Discharge Planner will work with Level I Inpatient Providers to review presenting clinical information, identify resources for successful discharge to the community, and monitor TXIX adult GMH/SA Behavioral Health Recipients (BHRs) follow up with the outpatient provider. The Discharge Planner works under the supervision of the Health Center Director or Site ManagerDuties:Discharge Planner activities will be performed in both inpatient and outpatient settings.The Discharge Planner will connect individuals receiving Level I inpatient services with ongoing TERROS Outpatient treatment upon discharge.The Discharge Planner will work with BHRs while in the hospital, will work with transitioning BHRs, the hospital unit staff, the Outpatient team, family, RBHA care management, PCPs and Health Plans as clinically indicated to ensure effective transition to Outpatient or community services, reduce the risk of readmission to hospital, reduce the utilization of Crisis services and improve the coordination with the PCP and TERROS psychiatric provider.Must be able to work well and tactfully with individuals and groups.Confidentiality of client records shall be maintained during and after employment at TERROS.
