Job summary

Thomas Jefferson University Hospitals, Inc. is seeking a Discharge Planner to coordinate post-acute services, schedule follow-up appointments, and liaise with the transfer center to ensure timely, safe transitions from hospital to home or another care setting.

The role collaborates with care managers, providers, and payers to obtain authorizations, document activity in the medical record, and support patients and families through the discharge process.

Qualifications

• Bachelor’s Degree in a healthcare-related field required.
• 2 years in healthcare with <1 year as a new graduate expected.
• Knowledge of scheduling, HIPAA, Medicare/Medicaid regulations and medical terminology.

Responsibilities

• Coordinating post-acute services (home health, DME, transportation) with care managers and social workers.
• Obtaining payer authorization for level of care and post-discharge services; communicating denials.
• Documenting discharge planning activities in the medical record for compliance.
• Reviewing Medicare Notice of Hospital Discharge for admissions.
• Scheduling follow-up appointments and linking them to orders and payer requirements.
• Acting as liaison among patients, families, providers, payers and community resources.
