Job summary

Terros Health is seeking a Discharge Planner to coordinate transitions from inpatient to outpatient care at our 27th Ave location in Phoenix. The role involves linking patients to outpatient treatment after discharge and collaborating with hospital staff, care management, PCPs, and health plans to prevent readmission.

Ideal candidates hold a high school diploma or GED, or a relevant bachelor's degree with health experience, and possess solid computer skills, including Word, Excel, Outlook, and

Qualifications

• High School Diploma or GED.
• 4+ years combined behavioral health education and work experience.
• Bachelor's degree with 1 year behavioral health experience, or a bachelor's in behavioral health.
• Computer skills: Word, Excel, Outlook, EMR (NextGen) preferred.

Responsibilities

• Discharge planner activities in inpatient and outpatient settings.
• Connect individuals with ongoing outpatient treatment after discharge.
• Collaborate with hospital staff, outpatient team, families, RBHA care management, PCPs and Health Plans for smooth transitions.
• Maintain client confidentiality during and after employment.
• Must work well with individuals and groups.
