Job summary

Oceans Behavioral Hospital Waco is seeking a Discharge Planner to coordinate patient treatments with physicians, therapists and nursing staff, and to manage discharge planning for inpatient and outpatient programs.

You will review psychosocial data, complete discharge assessments, schedule appointments and ensure timely documentation, while collaborating with the care team to meet treatment needs and regulatory standards.

Qualifications

• Associate degree with emphasis in healthcare or bachelor’s degree in social services preferred.
• At least one year psychiatric/chemical dependency experience preferred.

Responsibilities

• Functions as patient liaison to coordinate treatments with physicians, therapists, and nursing staff.
• Reviews psychosocial data and discharge assessments for treatment team approval.
• Schedules appointments and follows up on absentees; completes continuing care plans.
