Job summary

Grady Health System in Atlanta, GA, seeks a Nursing Discharge Navigator to coordinate safe, timely discharges and arrange follow-up care. You will work with doctors and care teams to reconcile medications and provide education for patients and families.

The role emphasizes reducing readmissions, coordinating transportation, and ensuring smooth transitions for heart failure patients. GA RN licensed nurses with case management experience are preferred.

Qualifications

• Bachelor's degree in Nursing required.
• 3+ years of clinical nursing experience.
• Nurse case management experience preferred.
• GA RN license required.

Responsibilities

• Coordinate and schedule follow-up appointments with primary care providers and specialists prior to patient discharge.
• Conduct discharge phone calls to assess patient understanding and ensure post-discharge needs are met.
• Ensure prescriptions are reconciled, called in, and accessible to patients prior to discharge.
• Provide comprehensive discharge education including medications, symptoms, diet, weight monitoring, and when to seek care.
• Confirm transportation arrangements are secured prior to discharge.
• Collaborate with physicians, case management, pharmacy, social work, and therapy services to support transitions of care.
• Identify barriers that may delay discharge and address them early.
