Job summary

HCA Florida Woodmont Hospital is seeking a Case Management Assistant (PRN) to support discharge planning and post-acute referrals. You will work with the RN Case Manager or Social Worker to identify barriers to discharge and coordinate timely referrals, payor authorizations, and PAC placements.

You will handle clerical duties, coordinate multidisciplinary rounds, and assist in transferring patients to post-acute providers, including arranging outpatient dialysis when needed.

Qualifications

• Two years of experience in a health care setting is required.
• Medical knowledge and terminology is required.
• CNA/EMT licensed or certified preferred.

Responsibilities

• Coordinates with RN Case Manager and Social Worker for support and updates on PAC referrals.
• Provides clerical support for multidisciplinary rounds and updates in case management systems.
• Makes referrals and coordinates with post-acute care providers and transports as needed.
• Schedules outpatient hemodialysis appointments and monitors PAC referrals for timely placement.
• Communicates status of referrals, bed availability, and barriers to ensure timely discharge.
