Qualifications

• Current Licensure per state guidelines in clinical or related discipline OR a Bachelor's or Master's in a health or human services discipline.
• Preferred: Previous experience in Case Management and Discharge Planning.
• CCM Certification preferred.

• Coordinate health care decisions to assure plan effectiveness and discharge planning.
• Manage care planning and coordination across the continuum.
