Job summary

Ideal candidates hold an Associate Degree in Nursing or a Master’s in Social Work with relevant experience and preferred credentials such as BSN, MSW, or Case Management Certification.

Qualifications

• Requires Associate Degree in Nursing or Masters in Social Work or related field.
• Florida RN licensure; new RN hires must obtain BSN within 3 years of hire.
• Preferred: BSN, MSW, and Case Management Certification.

Responsibilities

• Develop patient's plan of care and facilitate throughput.
• Conduct utilization review and promote appropriate utilization of resources.
• Identify needs and establish safe discharge plans.
• Collaborate with patient, caregivers, Attending Physician, Utilization Review Coordinator, and care team.
