HCA Florida Woodmont Hospital is seeking a Case Management Assistant (CMA) PRN to support discharge planning, coordinate post-acute referrals, and assist with patient flow under RN Case Manager or Social Worker supervision.

The CMA will process referrals, arrange post-acute services, and help with discharge documentation while maintaining patient privacy and compliance with hospital policies.

Qualifications

• Associate degree preferred.
• Medical knowledge and terminology required.
• Minimum of two years experience a health care setting required.
• Currently licensed/certified as a health care professional (CNA, EMT) preferred.

Responsabilités

• Coordinates with RN Case Manager and Social Worker for discharge planning and updates throughout the day.
• Provides clerical support of multidisciplinary rounds, including updating systems.
• Makes referrals through the web-based referral system to post-acute care providers and sends required medical records.
• Schedules outpatient hemodialysis appointments.
• Monitors PAC referrals and expedites placement.
• Prioritizes workload to ensure timely contact and intervention with PAC settings.
• Communicates status of PAC referrals and bed availability to the care team.
