Job summary

AdventHealth Palm Coast is seeking a qualified Care Management professional to support discharge planning across inpatient and ED settings. You will assess patients and families, coordinate post-acute services, and ensure complete medical records for smooth transitions.

The role emphasizes psychosocial assessment, strong communication, and collaboration with a multidisciplinary team to optimize patient outcomes and experience.

Qualifications

• Master's degree required.
• 2+ care management experience preferred.
• 2+ social work experience required.

Responsibilities

• Provides grief counseling, disease adjustment support, crisis intervention, goals of care planning support, and de-escalation services for patients as appropriate.
• Assess needs for discharge planning in inpatient, observation and/or emergency departments.
• Review medical records and integrate clinical, social, and financial factors into care plans.
• Develop discharge plans with contingency plans for timely care coordination.
• Coordinate with post-acute services to ensure continuity of care and complete transfer of records.
• Participate in multi-disciplinary rounds to identify resources for discharge and affordable care options.
• Document discharge planning evaluations, barriers, and patient rights during hospitalization.
• Coordinate with payors for authorization of post-acute care as needed.
• Assess readmission reasons for identified patients and families.
• Other duties as assigned.
