Qualifications

• Education: Bachelor’s Degree required.
• Experience: 2-3 years preferred in case management, utilization review, or discharge planning.
• Licenses: RN, PT, OT, SLP, RT or LCSW/LICSW required.

• Discusses assessment findings and patient status with physician and/or treatment team to maximize outcomes.
