Job summary

Ocean View is seeking a dedicated Case Manager/Discharge Planner to support discharge planning and coordinated care for patients in a psychiatric setting. You will work under the Director of Social Work to assess needs, arrange services, and connect families with community resources to improve outcomes.

Role involves collaboration with treatment teams, referrals, and documentation across inpatient and outpatient services.

Qualifications

• Bachelor's degree in psychology, human services, social work, or counseling from an accredited college or university.
• Two or more years direct service experience with severely and persistently mentally ill patients.
• Demonstrates competencies and knowledge in mental/behavioral health.
• Ability to speak, read, and write legibly in English.
• Remain calm and effective in high-stress or crisis situations.
• Strong teamwork and collaboration across disciplines and departments.
• Proficiency in computers and electronic documentation.
• Current CPR, BLS, and First Aid certification, or able to obtain within 90 days.

Responsibilities

• Coordinate discharge plans closely with the patient’s treatment team.
• Make accurate referrals for patient placement, case management, and aftercare services.
• Bridge the gap between hospital and referral sources for smooth care transitions.
• Maintain up-to-date knowledge of community resources.
• Work with intake staff to manage benefits, admissions, and discharge steps.
• Partner with social services to align patient care.
• Collaborate with physicians to ensure continuous care.
• Join treatment team meetings and provide feedback on patient plans.
• Write intake notes for new patients.
• Complete daily progress notes and weekly summaries on time.
• Handle paperwork and documentation tasks as assigned.
• Talk with families about admissions, discharges, and next steps.
• Support IOP/PHP programs based on census and needs.
