Job summary

The Center at CenterPlace is seeking a dedicated Case Manager to support patient placement referrals, assessments, care plans, and meticulous record-keeping. You will collaborate with multiple departments to ensure timely discharges and continuity of care for residents.

Responsibilities include psychosocial assessments, care planning, and compliance with federal and state regulations. This role is in-person, full-time, with competitive hourly pay and benefits.

Qualifications

• Minimum 1 year of case management experience.

Responsibilities

• Complete psychosocial assessments and maintain resident social histories.
• Coordinate discharge planning with hospitals, families, physicians, and insurers.
• Assist residents and families with discharge needs including home health and transportation.
• Provide education on Medicare/Medicaid, insurance benefits, and advance directives.
