Job summary

Virginia Commonwealth University Health Systems is seeking a Discharge Planner proficient in Medicare, Medicaid, and Commercial Insurance rules for nursing facility and skilled care placements post-hospitalization. You will identify community resources and collaborate with Healthcenter staff to ensure safe, timely discharge while maintaining high-quality patient interactions.

The role requires independent work, strong communication skills, and proficiency in computer-based tasks.

Qualifications

• Bachelor's Degree in related Human Services field from an accredited program. An equivalent combination of education, training, and/or directly related experience may be considered in lieu of the stated educational requirements.
• Education/training PREFERRED: Bachelor's Degree in Social Work from an accredited program.
• Experience REQUIRED: None
• Experience PREFERRED: Two (2+) or more years of experience in acute care or other healthcare setting with emphasis on care management/discharge planning

Responsibilities

• Discharge planning coordination with post-hospital care resources (community).
• Interact with multiple Healthcenter staff to arrange appropriate placement.
• Demonstrate critical thinking, independence, and ability to manage multiple priorities in a fast-paced environment.
