Job summary

Psychiatric Institute Of Washington is seeking a Discharge Planner to coordinate safe discharge processes from inpatient to community-based care. You will oversee discharge flow, ensure timely transitions, and collaborate with internal teams to arrange aftercare services.

The role requires 1+ year in a healthcare setting and strong Excel skills, with duties spanning coordination, documentation, and follow-up communications to support patient outcomes.

Qualifications

• High School Diploma or GED required.
• 1+ years of healthcare experience; behavioral health a plus.
• Proficient in Excel.

Responsibilities

• Assist discharge planning with aftercare arrangements, including referrals, scheduling follow-ups and transportation.
• Assist with patient satisfaction and clinical outcomes surveys (CABA/BASIS).
• Initiate follow-up calls to external referral sources and manage care point alerts.
• Track discharge data, compile reports, and audit discharge paperwork for accuracy before transition.
• Ensure discharge transition records are sent to receiving providers.
• Audit and confirm discharge paperwork accuracy prior to patient transition home or to a community program.
• Perform other duties as assigned by this position.
