Job summary

Qualifications

• High School education or GED required.
• Associate's/Technical degree preferred.
• Two years' experience in an acute or long-term medical setting preferred.

Responsibilities

• Demonstrates initiative in managing the discharge/transition process.
• Composes confidential correspondences, reports, and related material.
• Supports physicians by providing information to referral agencies and readiness for discharge.
• Acts as the discharge planning contact for the patient and family.
• Learns processes to provide adequate support when others are absent.
• Performs other job-related duties as assigned.
