Job summary

Qualifications

• BSN degree preferred.
• RN licensure in the state of practice required.
• Care management experience preferred.
• BLS certification required.

Responsibilities

• Leads coordination of patient care with the care team, ensuring timely and appropriate care.
• Ensures the interdisciplinary care plan aligns with the patient’s clinical course and needs.
• Discusses resource utilization, treatment plan, LOS, and discharge planning with physicians.
• Coordinates transfer of patients to tertiary centers when needed.
• Provides oversight and data collection for regulatory and accreditation compliance.
• Performs other job-related duties as assigned.
