Job summary

Responsibilities

• Provide grief counseling, disease adjustment support, crisis intervention, goals of care planning, and de-escalation for patients as appropriate.
• Assess discharge planning needs across inpatient, observation, and emergency departments.
• Review medical records to inform transition of care plans.
• Develop discharge plans with contingency measures for timely care coordination.
• Communicate with post-acute services to ensure continuity of care and complete medical records.
• Participate in multidisciplinary rounds to review patient status and discharge plans.
• Educate patients and families on illness impacts and mobilize resources while supporting patient empowerment.
• Organize and facilitate patient/family care conferences with the team.
• Document evaluations, discharge plans, and barriers to progression.
• Advocate for patient rights and coordinate with payors for post-acute authorization.
• Assess readmissions for root causes and strategies.
