Qualifications

Responsibilities

• Coordinate discharge plans with case managers, patients, families and providers to facilitate safe post-acute placements.
• Verify insurance and process referrals post-discharge; communicate progress and reschedule appointments as needed.
• Provide patients with written/verbal instructions for pre-operative tests and follow-up.
• Determine patient eligibility per program/contract standards and resolve related issues.
• Follow HIPAA procedures and prepare reports on discharge activity.
