Qualifications

Responsibilities

• Arrange post-discharge services.
• Create and follow up on electronic referrals using the Case Management system.
• Review patient choice letters with patients/families for signatures.
• Provide follow-up information to Medicare patients before discharge.
• Communicate with patients, families and care team members.
• Complete tasks assigned by the Case Manager.
• Make copies, send faxes and make phone calls.
• Participate in process reviews or audits.
• Other duties as assigned.
