• Monitors progress against discharge plan with care team and documents in required programs.
• Provides resources to patients and families to meet payor documentation requirements.
• Responds quickly to issues; informs Case Managers and Social Workers of complex cases.
• Performs duties in alignment with hospital values and mission.
