Responsibilities

• Coordinate patient care across the continuum from pre-admission through post-discharge planning.
• Collaborate with physicians, nurses, and community agencies to identify and arrange for appropriate care.
• Assess patient needs, manage utilization, and optimize clinical outcomes to promote safe discharges.
• Review assessments and plans to ensure appropriate admission status and level of care.
• Document care plans and act as a resource for patients and families while maintaining confidentiality.
