Job summary

AMM is seeking an Outpatient Care Coordinator in Long Beach, CA to support day-to-day care management operations and ensure timely follow-up after hospital or facility discharge.

You will coordinate non-clinical activities, communicate with members and providers, and document outreach accurately in the care management system. This role emphasizes patient engagement, resource coordination, and adherence to HIPAA/privacy rules.

Qualifications

• High school diploma or equivalent required; associate degree or additional healthcare education preferred.
• Experience in healthcare, managed care, or care management preferred.
• Proficiency with Microsoft Office and ability to learn healthcare software systems.

Responsibilities

• Coordinate non-clinical care management activities and communicate with members, providers, and teams.
• Monitor discharges and assist with timely post-discharge follow-up.
• Assist with intake, enrollment, outreach, and routing of clinical needs to Case Managers.
