BayCare in Clearwater, FL, is seeking a Social Services Specialist for PRN hourly coverage at Morton Plant Hospital. The role centers on collaborative discharge planning, coordination with care teams, and linking patients to community resources. A Bachelor's degree in Social Work or related field is required, with internship or prior social work experience in acute care preferred.

Certifications such as ACM or CCM are preferred. Schedule: 8:00 AM - 4:30 PM, every 3rd weekend, with no on-call.

Qualifications

• Required Bachelor's in Social Work, Sociology, or Related Field
• Required 6 months Internship or 6 months Social Work in an acute care setting
• Preferred ACM (Case Management) or CCM (Case Manager) certification

Responsabilités

• Being a member of a collaborative practice model with patients, nurses, hospital care coordinators, physicians, and other practitioners, caregivers, and the community
• Coordinates, refers and plans discharge in collaboration with care teams
• Considers treatment, social and economic implications for patients and families based on age and discharge needs
• Understands discharge planning and links with community resources
• Plans, organizes and manages multiple functions effectively
