Job summary

AdventHealth in Tampa, FL is seeking a part-time Care Manager to support discharge planning, care coordination, and patient advocacy. The role emphasizes collaboration with a multidisciplinary team to optimize patient outcomes and ensure timely transitions.

The successful candidate will have a Master’s degree, 2+ years in care management or social work, and strong communication skills. Part-time schedule with Day-Weekend shifts, located in Florida.

Qualifications

• Master's degree required.
• 2+ care management experience preferred.
• 2+ social work experience required.

Responsibilities

• Provides grief counseling, disease adjustment support, crisis intervention, goals of care planning support, and de-escalation services for patients as appropriate.
• Assesses discharge planning needs for inpatient/outpatient settings to avoid delays in discharge.
• Reviews medical records to incorporate clinical, social, and financial factors into care plans.
• Develops discharge plans with contingency plans to ensure timely care coordination.
• Coordinates communication with post-acute care services for continuity of care and accurate records.
• Participates in multi-disciplinary rounds to identify discharge resources and address delays.
• Educates patients and families about illness, impacts, and resources; advocates for patient rights.
• Organizes family care conferences with the care team.
• Documents evaluation, plans, and barriers to progression; tracks patient needs.
• Advocates for patients and liaises with payors for post-acute authorization.
• Assesses readmissions for underlying reasons and opportunities for improvement.
