Qualifications

• Master’s degree in Social Work, Mental Health, Psychology or Marriage and Family Therapy is required.
• Two years of direct clinical experience with emphasis on the population served; internship may substitute.
• Handle with Care (HWC) Certification required for Behavioral Health Unit.

Responsibilities

• Lead continuity of care across inpatient, ED and ambulatory settings to ensure integrated discharge planning and follow-up.
• Develop relationships with patients and families to support collaboration and timely discharge planning.
• Monitor progress toward discharge plans and adjust as patient condition changes.
• Educate patients and families about self-management and access to health care resources.
• Coordinate with IT systems to support registry reporting and population health initiatives.
