Job summary

BayCare in Clearwater, FL, is seeking a Social Services Specialist for PRN hourly coverage at Morton Plant Hospital. • Coordinates, refers and plans discharge in collaboration with care teams
• Considers treatment, social and economic implications for patients and families based on age and discharge needs
• Understands discharge planning and links with community resources
• Plans, organizes and manages multiple functions effectively

The Social Services Specialist responsibilities include:

• Being a member of a collaborative practice model that includes patients, nurses, hospital care coordinators, physicians, and other practitioners, caregivers, and the community


• Provides coordination, referral and interdisciplinary discharge planning


• They are knowledgeable about treatment, social and economic implications for individuals and families as it pertains to their age, stage of development and discharge needs


• Possesses a clear understanding of discharge and transition planning and linkage with community resources


• Demonstrates skills in planning, organizing, and managing multiple functions

Position details:

• Location: Healthy Families Program -Morton Plant Hospital - Clearwater, FL


• Status: PRN (as needed), hourly


• Schedule: 8:00 AM - 4:30 PM


• Weekends: Availability for every 3rd weekend


• On Call: No
