Job summary

Emanate Health is seeking a Case Management Coordinator to support discharge planning and clerical tasks in a fast-paced health system. You will arrange ambulance transport, communicate with skilled nursing facilities, home health agencies, and equipment providers, and document activities in Meditech and BAR to ensure smooth patient transitions.

Minimum education is a high school diploma with preferred clerical experience and strong Microsoft Office skills.

Qualifications

• High School diploma or equivalent preferred.
• One year general office/clerical experience preferred.
• Proficient in Microsoft Office.
• Knowledge of medical terminology preferred.
• Excellent customer service and organizational skills required.

Responsibilities

• Assist with clerical needs of discharge planning under supervision.
• Fax inquiries and CCS referrals; schedule ambulances and coordinate with facilities.
• Document activities in BAR and Meditech as required.
• Check voicemail, retrieve orders, and distribute to staff.
